Patient Instructions for Medical Record Requests

Capital Digestive Care has partnered with HealthMark Group to ensure the accurate and timely completion of medical record requests.

Does my office participate and how do I request my records?
If you are a patient at most of our practices, requests may be submitted electronically to HealthMark’s Request Manager at https://requestmanager.healthmark-group.com. Once logged in, select “Submit Request” from the menu options and enter all required fields to provide an authorization directly to HealthMark. Your medical record request will be processed, and a notification will be sent via mail or email once complete and available for download.

Patients have two options when requesting medical records:

  1. You may request a copy of your medical records to be sent directly to another medical office.
  2. You may request a copy of your medical records be sent directly to you.

If you are a patient at one of our locations below, you will need to call the office to request your medical records over the phone:

  • Chesapeake, VA
  • Columbia, MD
  • Laurel, MD
  • Newport News, VA
  • Norfolk, VA
  • Williamsburg, VA

NOTE: It is the patient’s responsibility to request outside records be sent to our office. Please contact the providing facility for instructions. All patient requests for records will be processed by HealthMark Group.
